One hundred and fifteen patients, January 2006 to June 2012: the forehead shortened through a trichophytic incision, with the scalp flap freed widely so that the closure carries little tension.
What was recorded
One hundred and fifteen patients operated on between January 2006 and June 2012. Before surgery a gliding test was made to judge how far the forehead skin would move; a zigzag line was designed and the skin excised through a trichophytic incision; the scalp flap was dissected in the subgaleal plane, freed well beyond the defect and fixed with an absorbable device rather than pulled into place. The forehead was shortened by 7 to 20 mm, 14.19 mm on average.
What it shows
How much tension a closure carries is decided before it is closed — by how far the flap is freed and by what holds it while it heals. That is what keeps the line at the hairline quiet, and it is the same reasoning as the rest of this practice: the plane and the release first, the incision second.
What it does not show
Forehead shortening lowers the hairline; it is not the endoscopic forehead lift, which repositions the brow, and this paper does not evaluate that operation or say which patient needs which. The series is the clinic’s own, reported without a control group and with no long-term scar assessment, in a journal indexed on KCI and not on PubMed. The flap is freed so that the closure need not pull.
